- ✪ ✪ ✪ ✪ ✪I was referred to Dr. Shultz from the ER because my 7 year old broke her wrist. I'm giving him a one star rating because i could not give a lower score. A little history i adopted my daughter and have not quite got all her information switched to our names so i do not have her insurance card or a tangible copy of the card. This office has refused to treat my daughter based on this aspect even though i can provide all the proof i needed other then that card.
